The icon appears in the menu bar. There is no main window.
macOS asks once. Open System Settings → Privacy & Security → Accessibility and switch Tap2Click on. That is the only way a macOS app is allowed to send a click. If the toggle is on but the icon still shows a warning, pick Grant Accessibility permission → Reset & ask again in the menu.
A light touch on the Magic Mouse is a click. Two fingers is a right-click. Done.
Every build is signed with a Developer ID and notarized by Apple. Paranoid? Check it yourself.

Quit Tap2Click from its menu, drag Tap2Click.app from Applications to the Trash, and remove it from the Accessibility list if you like. Preferences live in ~/Library/Preferences/com.michael.tap2click.plist. Homebrew users: brew uninstall --zap tap2click.
